During the 7-8 hours of sleep, facial skin undergoes unique biomechanical and physiological changes that lead to clinically verifiable structural deformations\u2014commonly known as sleep creases.<br>\u200bSkin Viscoelasticity and Sustained Compression<br>\u200bClinical biology defines the skin as a viscoelastic tissue, capable of absorbing mechanical stress in the short term, but subject to permanent deformation in the extracellular matrix (ECM) under sustained pressure. During lateral or prone sleep positions, static pressure and shear forces exerted on facial tissues disrupt the structure over several hours. The collagen and elastin fibers suffer micro-damage from continuous compression, leading over time to the weakening of the dermal network and the permanence of characteristic vertical creases.<br>\u200bThe Skin&#8217;s Circadian Rhythm and Increased Permeability<br>\u200bRecent chronomolecular research proves that the skin responds to the circadian (daily) rhythm at a cellular level. At night, cell proliferation (cell division) accelerates; however, simultaneously, the skin&#8217;s barrier function weakens significantly. Transepidermal water loss (TEWL) reaches its peak during the nocturnal hours. A clinically dehydrated stratum corneum (the outermost layer of the epidermis) is much less capable of absorbing mechanical stress, allowing physical creases to damage the tissues more deeply.<br>\u200bHemodynamics and Transient Nocturnal Edema<br>\u200bIn a horizontal position, the dynamics of lymphatic drainage and microcirculation change. Interstitial fluid can accumulate in the facial connective tissues overnight (transient edema).
